Simply follow the five steps below...

1. OPEN A RELIANCE BANK ACCOUNT
     Visit any one of our 20 conveniently located branches and our friendly Reliance Bank staff
     can assist with choosing the account that is right for you.

2. STOP USING YOUR OLD BANK ACCOUNT
     Let all of the outstanding checks written against the account clear. In the meantime, destroy
     your ATM/Debit cards, deposit slips, and unused checks. Don't own a shredder?
     No problem, we'll even shred them for you!

3. CHANGE YOUR DIRECT DEPOSITS
     If you receive a direct deposit from your employer or a retirement plan, fill out the
     Direct Deposit Authorization Form and attach a voided check from your new Reliance Bank
     account to the form. Give the completed form to the business or organization issuing the
     direct deposit. For Social Security benefits please call 1-800-772-1213 to change your
     direct deposit.

4. CHANGE YOUR AUTOMATIC PAYMENTS
     Once you are certain that your direct deposits have been changed successfully, fill out the
     Automatic Payment Form for any automatic withdrawals and automated payment services
     you have arranged, including those using your old debit card number such as automatic
     payments made online. Mail the completed form to the business or organization automatically
     debiting your account.

5. CLOSE YOUR OLD ACCOUNT
     Once all of the automated transactions are converted and the outstanding checks have
     cleared, fill out the Close Account Request Form and mail it to your old bank.



The Direct Deposit Authorization Form, Automatic Payment Form, and Close Account Request Form require Adobe Acrobat Reader. With the help of Adobe Reader, you can fill in the form using your computer and print the completed form ready for your signature. If you do not have a recent version of Adobe Reader or are not sure, you may get a copy free of charge from the Adobe Reader download page.
